Welcome to the Vaccines for Children (VFC) Program provided by St.Charles Wellness Center. We are committed to providing high-quality immunization services to uninsured children in our community, and we believe that the VFC program is an important part of achieving that goal. Immunizations protect children from serious diseases and illnesses. They also help reduce the spread of disease in the community. It’s important to make sure your child is up-to-date on their immunizations.
What vaccines are available through the VFC program?
The VFC program provides vaccines that are recommended by the CDC’s Advisory Committee on Immunization Practices (ACIP). These vaccines protect against a range of diseases, including:
Measles, mumps, and rubella (MMR)
Hepatitis B
Varicella (chickenpox)
Human papillomavirus (HPV)
Tetanus, diphtheria, and pertussis (Tdap)
Pneumococcal disease
Haemophilus influenzae type b (Hib)
Hepatitis A (HepA) vaccine
Meningococcal (MenACWY, MenB) vaccine
Polio (IPV) vaccine

HOW TO OBTAIN A FLORIDA VACCINE RECORD (FORM DH 680)
The Form DH 680, Florida Certification of Immunization, must be used to document receipt of Immunizations and is required for enrollment into the Florida school systems, programs, childcare facilities, and family daycare homes.
Where can I go to obtain Form DH 680 ?
At St.Charles Wellness Center we will provide you with Form DH 680, The Parent PIN, State IMM Id and update immunization records into Florida SHOTS.
After your visit, parents can log on to Florida SHOTS secure server directly and print electronically certified copies of DH Form 680 from their home computers. We will provide you with the State IMM Id and Certification PIN of your child’s Form DH 680.
